The Case Manager will be asked to provide Case Management directly to the assigned case load. Besides, the incumbent will have to develop and implement the IP (Industrialized Plan), coordinate and monitor the process of implementation of services, and support written in the IP. The chosen candidate will also be required to help individuals and their families in acquiring services. The Case Manager will help connect each family with a variety of resources that are the most important.
Requirements
The Case Manager is required to have the following qualifications and skills:
- Bachelor's Degree in the related field highly required.
- Bachelor's Degree in Social Work / Human Behaviour Sciences / Human Services desired.
- Prior professional experience working with people, who suffer from developmental disabilities as well as providing support for their families.
